The sphere of healthcare is rather difficult to manage properly mainly because there are always some ethical issues that need to be resolved. It is almost impossible to organize this particular sphere in a way that would satisfy the requirements of all people. The reforms of healthcare called 'Obamacare' also include some ethical challenges that are still not addressed properly. The primal aim of the reform was to provide all people in the society with appropriate medical support and give a possibility to receive equal medical help. At the same time, there is a serious problem related to American medicine in general, which is very expensive and costs a lot to the state. The USA spend more on the sphere of health care than any other nation, but American people are not becoming healthier than those from other countries. While Obamacare has a primal aim to provide the medical service for all people in America, it is necessary to avoid critical increasing of the wastes. After the reform of health care became legal, the hospitals received certain amount of money for treating each specific condition of the patient (for example, pneumonia or cancer). The hospitals will not have a possibility to exceed the money they received for treating this condition which might theoretically lead to under-treatment. Specifically, the patient might receive less powerful or effective medicine, procedure, or treatment because it is generally cheaper than more effective one. However, such treatment might be not appropriate enough and lead to rather challenging consequences. At the same time, doctors are forced to resolve such ethical issues every day, and if they spend all money on one patient, they will not be able to help to the other people.

At the same time, along with new financial regulation in hospitals, there is still a possibility for patients to pay the doctors with cash and this information will not be included into their insurance. Such case also causes a lot of ethical challenges and problems for medical workers as it might lead to the situation that is completely opposite to the previous one. The doctor, who knows that the patient will pay with cash might practice over-care, for instance, by sending patient to the procedures that are not necessary for him, but might cause additional income for the doctor or the hospital he works in. Those, who does not want to go to another doctor, will be ready to pay for procedures without making a notion about their real aim. At the same time, doctors will have to choose between ethics and income.

Apart from the above mentioned, there might be strong ethical issues related to the patients, who are not appropriate to the standards and criteria established by the government. Having specific instructions related to the patients, the doctors might ignore certain cases that are obviously unusual or demand additional treatment, care, or some specific procedures. This is rather difficult ethical challenge for the doctors, who want to provide everybody with proper help, but have limited possibilities in this aspect. All the related ethical challenges are mainly concentrated on the general low effectiveness of the health care sphere and the necessity to limit the costs for it strictly while providing patients with the treatment.
